If appointments are to be cancelled or rescheduled please allow 24 hours notice. We reserve the right to charge a late cancellation fee of $35 for appointments not cancelled within this time period.
Payment is expected at time of service unless prior arrangements have been made. We accept cash, check, Visa, Mastercard, and American Express.
We are Delta Dental Insurance providers and can provide claim forms for all other insurance companies.
